Sections 1, 2, and 3 of the Application Form for the Individual Assessment of Educational Qualifications

Section 1: Confirmation of Applicant’s Understanding
The purpose of Section 1 of the Application Form for the Individual Assessment of Educational Qualifications is to confirm that you understand that:
- You are to provide truthful information in your Application Form for the Individual Assessment of Educational Qualifications and any supporting documents.
- There may be consequences for providing false or misleading information in your Application Form for the Individual Assessment of Educational Qualifications or in relation to your Application Form for the Individual Assessment of Educational Qualifications.
There are three statements. To confirm your understanding of the statements and to agree to the terms of the Application Form for the Individual Assessment of Educational Qualifications, check the box for each statement. Also type/print your name and indicate the date. The College treats this entry as your signature.
Section 2: Previous Applications
In the first part of Section 2, you are to provide information about any previous applications you made to the College.*

- If not, answer “no” to this question and leave the remaining questions in Section 2 blank.
- If yes, answer “yes” to this question and complete the rest of Section 2.
- Application reference number – Provide the application number that you were given when you previously applied for registration with the College.
- First and last name you applied under – Provide the first and last name you used on your previous application to the College.
- If your name has changed since then, do not enter it here. You will provide your new name in Section 5 of the form.
In the second part of Section 2, you are to provide information about any previous membership you may have had with the College.*
*Are you a former member of the College whose Certificate of Registration was revoked?

- If you are a former member whose Certificate of Registration was not revoked, answer “no” to this question and leave the rest of this part of Section 2 blank.
- If you are a former member whose Certificate of Registration was revoked by the College at any time in the past, answer “yes” to this question and complete the rest of Section 2.
- If you are a former member whose Certificate of Registration was not revoked, answer “no” to this question and leave the rest of this part of Section 2 blank.
- If you are a former member whose Certificate of Registration was revoked by the College at any time in the past, answer “yes” to this question and complete the rest of Section 2.
- Registration number – This is a unique, five-digit number assigned to your Certificate of Registration. It is also sometimes referred to as the College of Early Childhood Educators number (CECE#), or membership number. The number can be found on the College’s public register.
Section 3: Application Package
Section 3 of the General Application Form provides some important information related to the:
- Items you must submit directly to the College in order for the College to open an application file for you and begin the application process.
- Supporting documents you will need to arrange to be sent directly from a third party (e.g., another institution) to the College.
- Other supporting documents you, or a third party (e.g., another institution), may need to submit depending on your circumstances.
