Vous devez remplir chaque section en entier. L&rsquo;Ordre vous renverra les formulaires et\/ou votre paiement si ces derniers sont incomplets.<\/em><\/p>\n<p>Les frais d\u00e9pendent de la situation de chacun.*<\/p>\n<p><em>* Quels sont les frais correspondant \u00e0 ma situation?<\/em><\/p>\n<ul>\n<li>Pour obtenir la liste des frais\u00a0<a href=\"https:\/\/www.college-ece.ca\/fr\/demandeurs\/section-4-liste-des-frais-de-dossier-et-droits-dinscription-2\/\" target=\"_blank\" rel=\"noopener\"><span class=\"ms-rteStyle-Emphasis\">de dossier et\u00a0frais d&rsquo;adh\u00e9sion, consultez cette page.\u200b<\/span><\/a><\/li>\n<\/ul>\n<p>Dans le\u00a0<em>Formulaire de demande d&rsquo;inscription dans le cadre d&rsquo;une \u00e9valuation individuelle des dipl\u00f4mes et de la formation<\/em>, les modes de paiement<em>\u00a0<\/em>sont class\u00e9s en trois groupes. Cochez la case indiquant comment vous paierez vos frais de dossier et\u00a0frais d&rsquo;adh\u00e9sion\u00a0et donnez les pr\u00e9cisions suppl\u00e9mentaires requises correspondant \u00e0 ce choix de mode de paiement.<\/p>\n<p><strong>1. Ch\u00e8que\/mandat-poste\/traite bancaire libell\u00e9(e) \u00e0 l&rsquo;ordre de l&rsquo;Ordre des \u00e9ducatrices et des \u00e9ducateurs de la petite enfance<\/strong><\/p>\n<ul>\n<li>Choisissez cette option si vous payez par ch\u00e8que, par mandat-poste ou par traite bancaire.<\/li>\n<li>Les frais de dossier et les frais d&rsquo;inscription appara\u00eetront comme deux transactions distinctes sur votre relev\u00e9 de carte de cr\u00e9dit\u200b.\u200b<\/li>\n<li>Sur le Formulaire de demande d&rsquo;inscription dans le cadre d&rsquo;une \u00e9valuation individuelle des dipl\u00f4mes et de la formation, indiquez :\n<ul>\n<li>le num\u00e9ro de ch\u00e8que, du mandat-poste ou de la traite bancaire, <strong>et<\/strong><\/li>\n<li>le montant pay\u00e9 en dollars canadiens.<\/li>\n<\/ul>\n<\/li>\n<li>Les instructions et exigences relatives au paiement par ch\u00e8que, mandat-poste ou traite bancaire figurent dans la section <a href=\"https:\/\/www.college-ece.ca\/fr\/demandeurs\/exigence-no-3-payer-les-frais-de-dossier-et-droits-dinscription-2\/\" target=\"_blank\" rel=\"noopener\"><em>Exigence no 3 \u200b: Payer les frais de dossier et frais d&rsquo;adh\u00e9sion<\/em>\u200b,<\/a> du pr\u00e9sent guide.<\/li>\n<\/ul>\n<p><strong>2. \u200bPar carte Visa, Visa D\u00e9bit ou MasterCard\u200b<\/strong><\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>Si vous payez par carte Visa, Visa D\u00e9bit ou MasterCard, cochez la case correspondant au type de carte utilis\u00e9.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Les cartes de cr\u00e9dit American Express et les cartes de cr\u00e9dit pr\u00e9pay\u00e9es ne sont pas accept\u00e9es. Les cartes de d\u00e9bit autres que Visa D\u00e9bit ne sont pas accept\u00e9es.<\/p>\n<ul>\n<li>Afin que l&rsquo;Ordre puisse d\u00e9biter votre carte Visa, MasterCard ou Visa D\u00e9bit, vous devez l&rsquo;autoriser \u00e0 le faire. Sur le Formulaire de demande d&rsquo;inscription dans le cadre d&rsquo;une \u00e9valuation individuelle des dipl\u00f4mes et de la formation, cochez la case situ\u00e9e \u00e0 c\u00f4t\u00e9 de la d\u00e9claration d&rsquo;autorisation et indiquez le montant, en dollars canadiens, qui doit \u00eatre pr\u00e9lev\u00e9 sur votre carte Visa, MasterCard ou Visa D\u00e9bit.<\/li>\n<li>Sur le Formulaire de demande d&rsquo;inscription dans le cadre d&rsquo;une \u00e9valuation individuelle des dipl\u00f4mes et de la formation\u200b, indiquez :\n<ul>\n<li>que vous autorisez l&rsquo;Ordre \u00e0 d\u00e9biter votre carte Visa, MasterCard ou Visa D\u00e9bit, comme pr\u00e9cis\u00e9 ci-dessus <strong>et<\/strong><\/li>\n<li>le nom du titulaire de la carte, tel qu&rsquo;il appara\u00eet vraiment sur la carte* <strong>et<\/strong><\/li>\n<li>le num\u00e9ro de carte complet <strong>et<\/strong><\/li>\n<li>la date d&rsquo;expiration de la carte <strong>et<\/strong><\/li>\n<li>la CVV (valeur de v\u00e9rification de carte).<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>* Et si aucun nom ne figure sur le devant de la carte Visa D\u00e9bit?<\/p>\n<ul>\n<li>Si vous effectuez votre paiement avec une carte Visa D\u00e9bit sur laquelle ne figure aucun nom, indiquez le nom du titulaire de la carte.<\/li>\n<\/ul>\n<p>S\u200bi vous payez par carte Visa, MasterCard ou Visa D\u00e9bit, assurez-vous d&rsquo;avoir donn\u00e9 toutes les autorisations et informations mentionn\u00e9es ci-dessus. En cas d&rsquo;information manquante, l&rsquo;Ordre ne traitera <strong>pas<\/strong> votre r\u00e8glement et vous retournera le formulaire qu&rsquo;il vous faudra finir de remplir. Cela retardera l&rsquo;ouverture de votre dossier d&rsquo;inscription.<\/p>\n<ul>\n<li>Cette partie ne s&rsquo;applique qu&rsquo;aux personnes qui payent par carte Visa, MasterCard ou Visa D\u00e9bit. Check the box that indicates how you will pay your application and registration fees and provide the additional details required for that payment option.<\/p><p><strong>1. Cheque \/ money order \/ bank draft made out to the College of Early Childhood Educators<\/strong><\/p><p>Important: In order for the College to open an application file for you, you must submit a completed Application Form for the Individual Assessment of Educational Qualifications and full payment of the application and registration fees. You must complete each section in full. The College will return forms that are not completely full and\/or if payment is not included.<strong>1. Review your General Application Form to ensure that it is complete and accurate.<\/strong><\/p><ul><li>Select this option if you will be paying by cheque, money order or bank draft.<\/li><li>On the Application Form for the Individual Assessment of Educational Qualifications, indicate the:<ul><li>cheque, money order, or bank draft number, <strong>and<\/strong><\/li><li>amount paid in Canadian dollars.<\/li><\/ul><\/li><li>Instructions and requirements for paying by cheque, money order, or bank draft are provided in the 'Requirement 3: Paying the Application and Registration Fees'\u200b section of the Application Guide for the Individual Assessment of Educational Qualifications.<\/li><\/ul><p><strong>2. Visa, Visa Debit, or MasterCard<\/strong><\/p><ul><li>If you will be paying by Visa, Visa Debit, or MasterCard, select the box that corresponds to your credit or debit card.<br \/>American Express and pre-paid credit cards are not accepted. Debit cards, other than Visa Debit, are not accepted.<\/li><li>In order for the College to charge your Visa or MasterCard credit card or Visa Debit card, you must authorize the College to do so. On the Application Form for the Individual Assessment of Educational Qualifications, check the box beside the authorization statement and enter the amount, in Canadian dollars, to be charged to your Visa or MasterCard credit card or Visa Debit card.<\/li><li>The application and registration fee will appear as two separate transactions on your credit card statement.\u200b<\/li><li>On the Application Form for the Individual Assessment of Educational Qualifications\u200b, indicate:<ul><li>your authorization of the College to charge your Visa or MasterCard credit card or Visa Debit card as described above, <strong>and<\/strong><\/li><li>the cardholder's name as it appears on the actual card,* <strong>and<\/strong><\/li><li>the full card number, <strong>and<\/strong><\/li><li>the card's expiry date, <strong>and<\/strong><\/li><li>the card's CVV (card verification value).<\/li><\/ul><\/li><\/ul><p>* What if there is no name on the front of the Visa Debit card?<\/p><ul><li>\u200bIf you are making payment by a Visa Debit card that does not have a name on the front of it, enter the card owner's name.<\/li><\/ul><p>If you are paying by Visa or MasterC\u200b\u200b\u200bard credit card or Visa Debit card, make sure that all of the above authorization and card information is provided. If any information is missing, the College will not process your fee and will return the form to you for completion.
