Pour en confirmer la compr\u00e9hension, et pour approuver les termes du Formulaire de demande d&rsquo;inscription dans le cadre d&rsquo;une \u00e9valuation individuelle des dipl\u00f4mes et de la formation, veuillez cocher la case correspondant \u00e0 chaque \u00e9nonc\u00e9. Veuillez \u00e9galement entrer\/\u00e9crire votre nom et indiquer la date. L&rsquo;Ordre consid\u00e8re ces entr\u00e9es comme votre signature.<\/p>\n<h4>Section 2 : Demandes ant\u00e9rieures<\/h4>\n<p>Dans la premi\u00e8re partie de la section 2, vous devez fournir des renseignements sur vos demandes d&rsquo;inscription ant\u00e9rieures aupr\u00e8s de l&rsquo;Ordre.*<br \/><em>* Avez-vous d\u00e9j\u00e0 effectu\u00e9 une demande d&rsquo;inscription \u00e0 l&rsquo;Ordre des \u00e9ducatrices et des \u00e9ducateurs de la petite enfance?\u200b<\/em><\/p>\n<ul>\n<li><em>Si non, r\u00e9pondez \u00ab non \u00bb \u00e0 cette question et ne r\u00e9pondez pas aux questions de la section 2.<\/em><\/li>\n<li><em>Si oui, r\u00e9pondez \u00ab oui \u00bb \u00e0 cette question et remplissez le reste de la section 2.<\/em><\/li>\n<li><strong>\u200bNum\u00e9ro de r\u00e9f\u00e9rence de la demande <\/strong>\u2013 Indiquez le num\u00e9ro de demande qui vous a \u00e9t\u00e9 donn\u00e9 lorsque vous aviez pr\u00e9sent\u00e9 votre demande d&rsquo;inscription \u00e0 l&rsquo;Ordre.<\/li>\n<li><strong>Nom et pr\u00e9nom utilis\u00e9s pour pr\u00e9senter votre demande<\/strong> \u2013 Indiquez le nom et le pr\u00e9nom que vous aviez utilis\u00e9s dans votre pr\u00e9c\u00e9dente demande d&rsquo;inscription \u00e0 l&rsquo;Ordre.<\/li>\n<li>Si votre nom a chang\u00e9 depuis, ne l&rsquo;indiquez<strong> pas<\/strong> ici. Vous indiquerez votre nouveau nom dans la section 5 du formulaire.<\/li>\n<\/ul>\n<p>Dans la deuxi\u00e8me partie de la section 2, vous devez fournir des renseignements sur vos \u00e9ventuelles inscriptions ant\u00e9rieures \u00e0 l&rsquo;Ordre.*<\/p>\n<p><em>* \u00cates-vous un(e) ancien(ne) membre de l&rsquo;Ordre dont le certificat d&rsquo;inscription a \u00e9t\u00e9 r\u00e9voqu\u00e9\u200b\u200b?<\/em><\/p>\n<ul>\n<li>Si vous \u00eates un(e) ancien(ne) membre dont le certificat d&rsquo;inscription n&rsquo;a pas \u00e9t\u00e9 r\u00e9voqu\u00e9, r\u00e9pondez \u00ab non \u00bb \u00e0 cette question et ne r\u00e9pondez pas au reste de la section 2.<\/li>\n<li>Si vous \u00eates un(e) ancien(ne) membre dont le certificat d&rsquo;inscription a d\u00e9j\u00e0 \u00e9t\u00e9 r\u00e9voqu\u00e9\u200b par l&rsquo;Ordre dans le pass\u00e9\u200b, r\u00e9pondez \u00ab oui \u00bb \u00e0 cette question et remplissez le reste de la section 2.<\/li>\n<li><strong>\u200bNum\u00e9ro d&rsquo;inscription<\/strong> \u2013 Il s&rsquo;agit d&rsquo;un num\u00e9ro unique \u00e0 cinq chiffres attribu\u00e9 \u00e0 votre certificat d&rsquo;inscription. Il est parfois d\u00e9sign\u00e9 sous les termes \u00ab\u00a0num\u00e9ro de l&rsquo;Ordre des \u00e9ducatrices et des \u00e9ducateurs de la petite enfance (n<sup>o<\/sup> d&rsquo;EPEI) \u00bb, ou de \u00ab num\u00e9ro d&rsquo;adh\u00e9sion \u00bb. To confirm your understanding of the statements and to agree to the terms of the Application Form for the Individual Assessment of Educational Qualifications, check the box for each statement.
